During the overnight third shift from 10/19/24 to 10/20/24, Correctional Police Lieutenant Angel Moreno was told an incarcerated person had torn a piece off a suicide blanket at Midstate Correctional Facility, the Dept Of Corrections reported. Moreno took the torn blanket but did not issue a new one, did not have the cell searched to confiscate the torn piece, and did not report to the Area Sergeant and Shift Commander. Synopsis as reported by the agency

During the overnight 3rd shift from 10/19 /24 to 10/20/24, it was reported to Midstate Correctional Facility’s Correctional Police Lieutenant (CPL) Angel Moreno that an incarcerated person had torn a piece off of their suicide blanket. An investigation revealed that after receiving this report, CPL Moreno took possession of the torn suicide blanket from the incarcerated person but failed to issue a new one. SCPO Moreno failed to have the cell searched in order to confiscate the torn piece of blanket and failed to report the incident to the Area Sergeant and Shift Commander. SCPO Moreno signed a settlement agreement on 12/30/24, for a 30-day suspension, a transfer to the 2nd Shift, and to remain free of major discipline for 2 years or risk demotion to the Correctional Police Sergeant title. CPL Moreno also received 4 weeks of retraining.

On January 5, 2024 a code to alert for medical attention was called. An Incarcerated Person had committed suicide while Lt. Moreno was on duty. After review of logbooks, body worn camera footage and Institutional Camera Footage, it was determined that the Incarcerated Person was hanging in his cell presumably deceased for about 7 hours before the code was called. There were several logs in the log book stating that tours of the unit were completed and that the unit was "secure" however review of the cameras revealed no tours were actually completed. It was shown that Lt. Moreno was only in the unit for one minute which is not enough time to do a complete tour of a unit. A settlement agreement on this matter was signed on July 18, 2024.

Moreno that an Incarcerated Person had tore off a piece of his suicide blanket. While the piece of blanket was retrieved, no further action was taken. This incident was not reported to the Sergeant or Shift Commander. Lt. Moreno did not confiscate the blanket, issue a new blanket, have the cell searched or do any of the necessary actions as per protocol. Lt. Moreno signed a settlement agreement on this matter on December 28, 2024.
